FORM PACKET
Includes: Emergency, liability, and photo waiver forms.
Please download and complete this form packet before arriving to camp on the first day.
- Bring them with you on the first day of camp

- Mail to: CUC Fab Lab, C/O Amanda, 1301 S. Goodwin Ave., Urbana, IL 61801.
The Medical and Emergency Contact and Waiver of Liability forms are required for participation in any camp held at the CU Community Fab Lab.
The Photo and Video Release form is optional, but we would appreciate you completing the form to make your intentions clear whether you grant permission to use photos or not.
We do not require proof of vaccination to attend camps.
